Wellhead Assembly Failures
BOP’s, master valves, flanged connections, mandrels, gaskets, hangers and spiders grab the majority of the headlines when problems are associated with wellhead configurations. It’s not uncommon for failures to springboard design modifications and innovations. Efficiency, minimizing downtime expenses, and safety improvements are all beneficiaries of evaluating performance. We have diagnosed the sources of many failures and participated in design and material changes.
Wellhead master valve connection failure contributed to a blowout. The project required break-out torque measurements, 3D measurement of the surface flatness, and gasket malleability studies.
Flange connections were broken and the surfaces separated. Washout occurred through the steel gasket. Foreign materials disrupted the gasket seating.
